I got my LMV driving license last year from Gurgaon, would like to share the procedure for the same.
Address for submitting the application: Ekal Seva Center, Ground Floor, Mini Secretariat, Gurgaon.
Documents accepted as residence proof: Copies of any two from the following:
(1). Ration Card
(2). Election Voter Id Card
(3). Electricity Bill
(4). BSNL Telephone Bill
(5). Water Bill
(6). Nationalized Bank Pass Book
(8). Gas connection
Age proof: Copy of any one of the following:
(1). School Certificate.
(3). PAN Card
(4). LIC Certificate.
Since I am not a permanent resident of Gurgaon, I had to open a PNB saving account as a proof of residence along with Gas connection copy.
For learner driving license, one need to submit the attested photo copies along with the learner driving license form. Pay the license and medical fee, get the medical done from Medical officer present in the mini secretariat. After this, submit the form and other documents to the clerk. He will verify the documents and if everything is fine, one can collect learner driving license after a week.
Once armed with a learner driving license, one need to wait for 42 days and then he can apply for permanent driving license. Procedure is same except that in place of medical examination, driving test needs to be passed. It is conducted between 2:00 pm to 3:30 pm from Monday to Friday, by Gurgaon Traffic Police inspector.
I am overall satisfied with my experience in getting a DL. They have made the whole process transparent and simple. There is negligible room for agents and hence corruption level is quite low, at least in Gurgaon.
